Realistic damage system/ bleeding out and bandages. Shots to arm/hand crippling reload, aim and accuracy. Shots to legs make you limp or fall over. (Player option to succumb to injuries if you don't want to gimp around the battlefield.)
Adrenaline system - I would love adrenline to kick in during a charge so I can possibly take a ball or two to non-vital organs before bayoneting the enemy and bleeding out.
One hit one kill would still be the norm but this would add some variety instead of always knowing 1 hit =1 kill. A realisitic damage system would also reward player accuracy on vitals/head shots.

A Good Resource for The Maryland Campaign
I'd like to see shotguns, whether single or double-barreled. Some early war Confederates used their hunting shotguns due to a lack of military weapons. I believe that a Confederate unit with shotguns would be interesting.
